Cardiovascular magnetic resonance with late gadolinium enhancement improves mortality prediction beyond echocardiography: a comparative effectiveness study
نویسندگان
چکیده
Background We tested the hypotheses that 1) ejection fraction (EF) assessed by cardiovascular magnetic resonance (CMR) is a stronger predictor of all cause mortality than EF assessed by echocardiography; and 2) CMR with late gadolinium enhancement (LGE) improves risk stratification over echocardiography EF in Cox regression models as determined by net reclassification improvement (NRI). Echocardiography with EF measurement remains the initial noninvasive imaging modality for evaluation of patients, and it is used to individualize medical, surgical, and device-based therapy. Yet, if CMR with LGE improves risk stratification compared to echocardiography, then CMR would demonstrate a potential to optimize care through improved patient selection for interventions and more efficient allocation of healthcare resources.

Effectiveness of late gadolinium enhancement to improve outcomes prediction in patients referred for cardiovascular magnetic resonance after echocardiography
BACKGROUND Echocardiography (echo) is a first line test to assess cardiac structure and function. It is not known if cardiovascular magnetic resonance (CMR) with late gadolinium enhancement (LGE) ordered during routine clinical practice in selected patients can add additional prognostic information after routine echo. We assessed whether CMR improves outcomes prediction after contemporaneous ec...
متن کاملCardiovascular Magnetic Resonance and prognosis in cardiac amyloidosis
BACKGROUND Cardiac involvement is common in amyloidosis and associated with a variably adverse outcome. We have previously shown that cardiovascular magnetic resonance (CMR) can assess deposition of amyloid protein in the myocardial interstitium. In this study we assessed the prognostic value of late gadolinium enhancement (LGE) and gadolinium kinetics in cardiac amyloidosis in a prospective lo...

Materials and Methods: The Cardiac Magnetic Resonance images of sixty patients with history of remote ST segment or non-ST segment elevation myocardial infarction were ...
متن کاملThe combined incremental prognostic value of left ventricular ejection fraction, late gadolinium enhancement and global circumferential strain assessed by cardiovascular magnetic resonance
Background Left ventricular ejection fraction (LVEF) is powerful predictor of mortality and is used to guide treatment decisions. It is however subject to limitations, particularly when measured using echocardiography, as is most commonly done. Additionally, many major cardiovascular events occur in patients typically adjudged to be at lower risk using echocardiography (LVEF >35%). Assessment o...
متن کاملLate gadolinium enhancement cardiovascular magnetic resonance in genotyped hypertrophic cardiomyopathy with normal phenotype
A 35 year-old asymptomatic Caucasian female with a family history of hypertrophic cardiomyopathy (HCM) was referred for cardiologic evaluation. The electrocardiogram and transthoracic echocardiogram were normal. Cardiovascular magnetic resonance (CMR) was performed for further assessment of myocardial function and presence of myocardial scar. CMR showed normal left ventricular systolic size, me...
